Admission
We would like to advise you to complete the pre-admission process 48 hours prior to the day of your surgery. This will ensure a smooth admission process on the day of your visit.
This can be done by:
- Visiting the practice
- Downloading the form in PDF format and send it electronically
- Logging on to our on-line pre-admission link
PRIVATE PATIENTS
Please be advised that private patients are required to settle their account in full on the day of their procedure. Private patients must contact Omuramba Day Theatre before admission to obtain an estimation of costs. Accounts may be settled in the following manner:
- Cash
- EFT
- Credit or Debit card
MEDICAL AID PATIENT
When visiting the practice, kindly bring along the following:
- Letter from your Physician of the scheduled procedure (if supplied by your Physician)
- Medical aid card of the patient/main member
- Identity document of the patient/main member and
- Authorization code from the medical aid. (Please verify with your medical aid if pre-authorization was obtained by your Physician)
Online pre admission:
- Kindly click on this link to open/download our pre-admission form on the website.
- Complete all required fields
- Fax it back to the fax number as listed on our website
- Attach the letter from the Physician of the scheduled procedure (if supplied)
- Fax a copy of your medical aid card as well as your identity document
- Once completed please fax to the following number 083 323 8990 or e-mail to accounts@odt.com.na
MEDICAL AID AUTHORISATION
To request authorization for your procedure, your medical aid will need the following information:
- Your membership number
- The name of the patient (dependent code)
- Date of admission
- Reason for admission (procedure being performed)
- The name of the attending doctor and his/her practice number.
ON-LINE PRE-ADMISSION
Welcome to our on-line pre-admission service. This service allows you to take care of this necessary procedure in the comfort and convenience of your home or office. Kindly complete this form at least 48 hours prior to admission.
When all required fields are completed, click on the “done” icon, and your form will automatically be emailed to us.
On the day of your admission kindly bring the following with:
- Medical aid card of the patient/main member
- Identity document of the patient/main member
- Your authorization code from the medical aid
